Patient Registration – by a Wound Care Nurse

The wound care nurse runs the Wound Image Program to receive and manage the wound pictures that come in for her patients.
Patients need to be Registered at a clinic to make the pictures of their wounds come to the wound care nurse's computer.
If the patient is sending in his own pictures, then the username has to be his 10 digit phone number. Otherwise use some sensible way of identifying them, Situations vary so much that hard-and-fast rules may not be helpful.
The data you enter in this screen remains only in your computer, it does not go onti the Internet.
